We reviewed retrospectively the neonatal records of an inner-city hospital from January 1979 through December 1990 to determine whether the health care afforded to infants with high transcutaneous bilirubin concentrations in the first week of life has improved over the past decade. Neonates with bilirubin concentrations in the first week of life of 220 micrograms/dL or greater were reviewed, and the following data were obtained: gestational age, birth weight, gender, method of testing, the presence or absence of clinical jaundice, and any treatment of hyperbilirubinemia. All neonates were followed up for at least 2 weeks. The response to treatment was recorded. These patients were compared with a similar cohort of neonates with values below 220 micrograms/dL. Overall, the 25 high bilirubin neonates were treated with exchange transfusion or phototherapy for a total of 205 days. The jaundiced neonates of the high and low bilirubin groups were similar in age, weight, sex, method of testing, and gestational age at the time of birth. The overall clinical manifestations of hyperbilirubinemia were similar in the high and low bilirubin groups; the only difference was a greater incidence of jaundice in the high bilirubin group. Neonates with high transcutaneous bilirubin concentrations in the first week of life who did not receive exchange transfusion were compared with a similar group of neonates with lower bilirubin values and only mild clinical jaundice. Despite more rigorous testing methods and more specific institutional protocols, neonates with high bilirubin concentrations in the first week of life do not appear to be receiving more appropriate therapy./* * Copyright (C) 2013-2015 RoboVM AB 648931e174
